Your complaint will be forwarded to a Food Service Sanitarian for follow-up. In order for us to complete a thorough investigation, please provide us with the following information.

1. Your name, address and phone number
2. Name, location and phone number of the establishment
3. Date and approximate time of the incident
4. Who was involved (for example, cook, cashier, wait staff)
5. Description of the problem
